•YOU HEREBY HOLD HARMLESS AND RELEASE ICE FROM ALL CLAIMS, DEMANDS AND CAUSES OF ACTION WHICH YOU, YOUR HEIRS, REPRESENTATIVES, EXECUTORS, ADMINISTRATORS OR ANY OTHER PERSONS ACTING ON YOUR BEHALF OR ON BEHALF OF YOUR ESTATE HAVE OR MAY HAVE BY REASON OF THIS AUTHORIZATION.

IP Address
